2026–2027 Technology Permission Slip

Before a device can be issued to students for school use, a parent or guardian must sign the Chromebook Policy Agreement. Students will not be allowed to use a Chromebook until this form is signed.

Parent/Guardian Responsibilities

The GTH Chromebook Policy, Procedures & Information Handbook serves as a resource for both students and parents. Parents and students must read this handbook and agree to comply with its requirements. I acknowledge that both my student and I agree to all terms outlined in the Green Tech High Charter Student Handbook. I understand my child is responsible for his actions with the technology resources owned by Green Tech High Charter School.

  • My student understands the importance of properly caring for their assigned device and agrees to notify their parent/guardian and the school's IT Department or Front Office immediately if the device is damaged, malfunctions, or is lost.
  • There will be a $40.00 repair fee for any damage beyond normal wear and tear that is not covered by the device insurance. Lost or damaged chargers that require replacement will incur a $25.00 replacement fee.
  • If the device is lost, the parent/guardian must complete a loss affidavit. A $200 replacement fee is required before a replacement device will be issued.
  • If the device is stolen, the parent/guardian must file a police report and submit a copy of the report to the school office. A $200 replacement fee is required before a replacement device will be issued.
  • If a student experiences a hardware or software issue with their assigned device, they should visit the Main Office or contact the IT Department at techsupport@greentechhigh.org for assistance.
  • My student has no expectation of privacy with any usage of the device. The school may, without prior notice or consent, log, supervise, access, view, monitor, and record use at any time and for any reason.

I understand that my student's internet activity will be monitored while using school-issued devices and accounts. Internet filtering will be in place to help block access to inappropriate content. However, no filtering system is completely effective, and some inappropriate content may still be accessible. My student is expected to use the internet responsibly and avoid accessing inappropriate websites or materials.

I understand that violations of this Chromebook Policy & Procedure Agreement or related school technology policies may result in disciplinary action, fines for damages or losses, and/or the loss or restriction of technology privileges, as determined by the school.

    Fill in every field and sign the slip. Prefer paper? Print it, fill it out by hand, then scan or take a clear photo of each page.

    After completing the PDF, please email it to both staff members below. A device cannot be issued until the signed slip is received.
