Athletic Forms
What every student-athlete needs on file before the first practice.
No student may try out, practice, or compete until his athletic paperwork is complete. A student who does not have a valid physical in PowerSchool and a completed sports packet will not be eligible to participate.
Paperwork for Athletic Clearance
- Physical on record with Green Tech within the last 12 months.
- Sport history update with health questions filled out.
- Parent participation waiver.
- Sport packets can be picked up from Mr. France, the athletic director.
Three Steps for the Online Form
- Step 1: Fill out the form online — agree to Adobe's Terms of Use and press the blue "Continue" button to begin. If a field is not fillable, skip it; it will autofill on completion.
- Step 2: Press the "Click to Sign" button once complete.
- Step 3: Verify your email by opening the link Adobe sends to the address used on the form (check Junk or Spam). Your submission is complete once verified, and a copy is emailed to you.
- On phones and tablets, turn the device horizontal to fill out the form.
Once He Makes the Team
Additional paperwork is reviewed with families covering season policies and procedures. The Student Athlete Academic Policy found in the Student Handbook is followed throughout the season.
